Pathways to Achievement is an organization providing various healthcare services to individuals residing in and around the Duluth, Minnesota area. Pathways is the sister corporation to TBI Residential and Community Services. Pathways has been providing Nursing and Counseling services to the individuals in the care of TBI. Pathways also has been providing certified Special Transportation and Skilled Home Healthcare services since 1996 and has been an integral part in the success and growth of TBI as an organization and part of the Duluth Community. Due to the effectiveness of Pathways Home Healthcare and STS-TBI has grown to service over 125 individuals and 33 houses they are able to call Home.
Our services have been so effective with TBI that we have decided to offer our Home Healthcare out in the community so you or your loved one can still call their current place of residence Home. Whether it be a private home or a cooperative, independent, assisted living facility- we are there to do anything and everything possible to ensure you stay where you want to be- Home.
